Patient Resources

Everything you need to prepare for your first visit with us.

Before Your Appointment

  • Arrive 15 minutes early to complete your new patient paperwork.
  • Bring a valid Driver's License and your Insurance Card.
  • Bring a complete list of your current medications and dosages.

During Your Visit — What to Expect

  • You will fill out new patient paperwork including: General Consent for Treatment, Insurance Agreement, Record Release Authorization, Office Policies, and HIPAA forms.
  • A consultation involves a thorough one-on-one review of your medical history, medications, and discussions with a medical assistant.
  • The specialist will explain your diagnosis, recommend a course of action, and elaborate on when it should take place — including treatment options, potential side effects, and further specialist referrals if deemed necessary.

Office Policies

Cancellation Policy

24-hour notice required. Infusion appointments cancelled with less than 24 hours notice may incur a $100 fee. Patients who no-show 3 consecutive times may be redirected to Beaumont Infusion Center.

Late Arrivals

Patients arriving 15+ minutes late for a scheduled visit may be rescheduled. We do our best to accommodate late arrivals without compromising care for other patients.

Insurance & Billing

Patients are responsible for obtaining referrals, paying co-pays and deductibles, and covering non-insured services. For billing questions, call (248) 243-3935.

HIPAA Privacy

Your health information is protected. We only share relevant info with family members or friends you have formally authorized on your HIPAA form.
